The "Form" module is an interactive feature that can be used to gather information about the reader, such as email addresses and phone numbers. This article will explain how to add a "Form" module to a Newsroom or article and tailor it to gather desired reader feedback and data.

Adding a form module

Open the Newsroom dropdown, select Homepage or Pages to select a specific Page.

In the sidebar, open the Interactive tab and then select Form.

Toggle the module from OFF to ON.
- Configure the Form details:
- Title - Enter the title of the form,
- Results to email - To receive the results, add an email the data is send to.
- Submit button text - Fill out the call to action for the submit button.
- Form completion message - Fill out the text which appears on the screen after the form has been submitted.

Form submit action - Enable this option and choose where the page should redirect to after the form has been submitted or if the visitor can download a file:

Internal page - Select a Newsroom page from the dropdown

External link - Paste a webpage into the field.
- Downloadable content - Click Select item to open the Asset Manager to select a document from or upload a new one.

Adding fields to the Form
After configuring the Form details add the form fields that will gather information from the reader.
- Click Add Form Element and select the Form Element from the dropdown.
- After choosing an element a detail section opens to set up the element.
- Title - Enter a title of the form element into the box provided, indicating to the reader what information to enter.
- Required - Enable Required, to make it mandatory to fill out the field before the reader can submit the form.
- Once ready select Save to confirm the changes, and the form element is added to the form.
- Repeat these steps as needed to create the form.
- Drag and drop the elements by holding the directional icon and dragging and dropping them in the preferred order.
- Click Save to confirm the changes.
Form element options
All elements in the Form module can be mixed and matched, to customize the type of information gathered from the reader.
Text Field
Use text field to provide basic information, like a name, phone number, email address, or short comment.
- Validation - Enable validate if the the information entered is a Phone number or E-mail. Selecting None allows any text to be entered in any form.
Text Area
Use Text area to provide open, unformatted feedback.
Radio Button, Check box, or Dropdown menu
- The Radio button element lets readers select one option from a list of pre-determined answers.
- The Check box is intended to ask the reader to make one or more choices from a list of pre-determined answers.
- The Dropdown menu is to ask the reader to make one choice from a drop-down menu selection of pre-determined answers.
Configuration details
- + icon - Click + to add answers
- X icon - Click x to delete answers
- Directional icon - use the directional icon to drag and drop the answers in the preferred order.

Orientation - Choose if the answers will appear with a horizontal or vertical orientation.
View and export results
- Click View form results to access the form submissions entered so far.
- This will also allow you to export the results in Excel (XLSX) format.
